Amplify Life, LLC is a HIPAA-covered entity. We provide cash-only medical wellness services and collect protected health information (PHI) — any data about your health, treatment, or payment that can identify you.

How We Use & Share Your PHI

We only use or share your PHI as allowed by law, including:

• Treatment: coordinate with your other providers.
• Payment: process your cash, card, or HSA/FSA payments.
• Operations: Improve care, staff training, and quality audits, with your consent.
• Marketing, research, or sharing with third parties only with your consent.
• Required by Law: public health, abuse reports, court orders

We never sell your PHI without written permission.

Our Duties:

• Keep your PHI private and secure.
